Eden Park Academy Enrollment Info

When applying for your child to attend Eden Park Academy, please download and print out all these forms, fill them out, and turn them in to our office. At the same time, we also need to see your child's birth certificate, social security card, and valid current immunizations.

All the above forms are due in the main office no later than March 2, 2012. The lottery will be held on April 2, 2012.
